form created with publisher not redirecting email upon submit

Created a simple five box form to compliment my site and posted to the internet. After you click the submit button, an error page is displayed. Any suggestions?

there is no difference between versions in this regard.
Using a form in Publisher (all versions) is
really very simple as long as the following steps are taken...
1. design the form.
2. set the form's processing properties.
3. have FP extensions installed.
4. publish the web in the http method.

the faq only covers publisher 2000 and 2002. I followed the instructions

in the faq and still no go... perhaps there is some special instructions
for pub 2003 (my host didn't give options on what frontpage extentions, just
to upload or not).
