Form Creation

Hello,
I would like to create a form, where there is instructions on the form
about what kind of information is expeced in a section and who is responsible
for the section and then allow the user to type in the required information
(usually) a paragraph. When the information is recieved from all users, I
would like to merge all responses into one document and provide that document
as a report.

In my final report, I don't want to print the instructions

Can this be done??

Probably ... with a lot of work.
I learned about a new on-line service that might be ideal for this kind of
situation.
It is called Circle Up:

Hi,
I prefer option #1 on this website, using a Userform to get information from
users:
http://gregmaxey.mvps.org/Repeating_Data.htm
If everyone is answering the same unique "Questions" form sequentially, then
you can append the data to the previous data in the same unique "Answers"
document.
If everyone is answering separate copies of the "Questions" form
concurrently, then you'll have to collate the data later from separate
copies of the "Answers" documents.
You could also google for any possible templates (polls?) that fit your
requirements.
