Form errors

I have used one of the FP2003 templates to create a web site. When I fill in
one of the forms I get the error:

FronPage Error

Webmaster: Please see the server's application event log for more details.

I am hosting this web site. In C:\WINDOWS\system32\Logfiles\W3SVC1 I found
the following error:

06:33:18 127.0.0.1 GET /favicon.ico 404

Is htis the right one?

How do I go about fixing this form submission error?

Does the form have link on it to a favicon, if so remove it.
