FORM HELP PLEASE

Folks,

I have created a form on our church website for a free book offer. But when
I complete the form nothing happens.

I would like the request to be emailed to myself and our minister.

Our hosting service has FP extensions installed.

Can someone take a look at the form and please advise what I maybe doing
wrong and offer help on how to correct this problem.

The church is very excited about being able to offer there books on line.

#1- Check with the hosting service to see if the FPSE are
configured for sending email. This is a common solution to
these problems.

#2- I highly recommend using a script to generate the emails
as opposed to the FP form handler. Most spam harvesters
would find 3 addresses on that 1 page to send spam to.

frontpage handler only sends the email to one person (can't be sent to multiple
people) and only to the email address for the site owner.
