Form No Longer Working

I'm using FrontPage 2002 and published a web with a form
that sends results to my email address. About a week ago
my form stopped working...that is, after hitting Submit
the form no longer sends the results to my email and also
does not jump to my confirmation page. Although I had
not done any updates to my site I republished to see if
that would fix the problem...no dice. I also asked my
host to re-install the frontpage extensions (which they
say they've done)...still not fixed.

Any advice?

After hitting submit I now get sent to...

http://www.tenyearslate.com/_vti_bin/shtml.exe/mailing-
list.htm

I don't know what shtml.exe is and can't find _vti_bin.
Is that something I'm missing? If so, how do I add those
to my site?

Any help would be great.

Did your host also re-setup the forms to email transport?
