Form not appearing in sent email

I created a form (.opt).Looks great when I fill in the
blanks and send as an email. But when it is received it is
totally blank. I'm sure it is something simple. Any ideas?

HI,

Is it a message form?

If so you need to copy the contents of "Edit compose Page"
into "Edit Read Page"
