Form not writing to Access db

I have built a form in FP, and created the database, and have gone through
it many times. Although I fill out the form, online, and it gives me the
confirmation page (as if everthing is alright), in the end, it doesn't ever
write to the database... (Access 2000)

Is there a checklist I can go through on this? Once I get this working on my
ISP's server, I hope to bring it in house... So, I kind of want to know how
to do this...

Any thoughts? Any experienced people have a checklist? Any concise articles?
