Form opening as email message

HI Everyone!
hope someone can help me.
We've been using outlook forms for quite a while and ever
so often, after they're sent, they are then received as an
email message entitled Untitled. I'm not sure why?
Any ideas?
When I click Design this form, I can see all the fields
etc ..the real form.
