Form problem

I have a problem with one of the forms on my page.
The subject of the confirmation email is shown as:
Data posted to form 1 of
http://www.bed-and-breakfast-travel.com/webservices/info-request.htm

I define the subject in the form as:
S-Email-Subject="BnB Website Information Request"

any idea where the problem may be? Other site forms work ok.
I am using FrontPage2000. If you want to look at the form it is at the
url above.
BTW - I did change the name of the form from FrontPage_Form1 to my own name.
Changed it back to Form1 with no difference.

Hi Larry,

Uncheck Form field name where it says Subject line:

You would check this where you would use a specific field input as the
subject.

Remove what you have under Reply-to-line: Keep Form field checked and put
Email in place. Republish and see what happens.

FrontPage renames the form to FrontPage_Form1 when you set up validation for
the fields.
