Form requesting info.

Hi, I would like to create an Outlook Form that I can email to
co-workers and they can just select the appropriate response with a
combo box or with misc. info. in a memo box. I have never used forms in
Outlook and just becoming familiar with them in Access. If someone
could point me in the right direction and maybe even show me some
examples, that would be great.

First thing you need to do is provide information on yoiur email
environment: Are you using Microsoft Exchange.

They are using Microsoft Mail.

And the issue is? The newsgroup interface you are using apparently does not
quote earlier messages in the thread, making your latest message so short on
detail that you risk not getting the answer you're looking for. Please take
