Form reverts to a previous version on 'approve'

I'm really struggling with this one:

I have an InfoPath form published to SPS Form library. User creates new form
then can revisit it during the current week to update as necessary.

The problem doesn't come in until a manager (any user in the manager roll)
comes along, opens the form and clicks Approve. The only thing the approve
button does is change the status of the form from not approved to approved
(this is used to filter results in SPS). The next time the form is opened, it
opens in a previous version of the InfoPath template.

I've removed all InfoPath form templates from my computer a number of ways,
first was to open InfoPath and in the Recently Used Forms list, I selected
any forms and Removed them. I also navigated to C:\Documents and
Settings\<your user folder>\Local Settings\Application
Data\Microsoft\InfoPath. I Deleted the folder 'FormCache1'.

That should have removed all cached copies I had.

I have tried to publish to a new sharepoint library. I've even published to
a separate physical sharepoint server but the same thing keeps happening.
I've used different computers to access the sharepoint library as well.

Any help anyone could offer would be greatly appreciated.
