Form/Template for non-Outlook users

Can a form or a template that I create in outlook be edited and returned to
me from a web-based email account? If so, how doI publish it to make it
available? I am trying to harvest the same detailed information from several
individuals, not all of whom have outlook.

No. It can be done with Access 2007 + Outlook 2007, though, using an HTML message (not a form). If you have an earlier version of Office, consider using a web page.
