Form Template in Word

I have created a template form in Word which our staff will use to fill-in,
save and e-mail to me. The problem is that when the form is saved, it's
saved with their changes. What I would like to do is have a prompt ask to
them re-name the document so my original remains blank. Is there a way for
me to do this?

I did create an actual template in word but the template is only on my PC. I
have the form in a shared folder so they just go into the file and make their
changes. Do I save the template in the shared folder as a template?

Yes, you need to save the doc as a template and then email them the template,
not the doc
or
They can open Word, open your .doc, then File Save As, Save As Type:
Document Template. Everytime they open word, File; New, On My Computer; The
template is in General Tab. It opens the template. When you close, it
automaticaaly asks you to save your document as a .doc
