Form to collect some data & allow free text?

I want to distribute a form which captures specific data in 2-3 fields, but
also allows the rest of the document in free format.

Using Word's FORM Fields. I have not been able to lock only part of the
field. Is it possible??

Android,

You will have to separate the protected and unprotected
sections of your document with a section break. Use
Tools>Protect Document to protect/unprotect applicable
form sections.
