Form to Rent / Return Books

I have been working on a database for the past few weeks
and have hit a total roadblock. I would like to create a
form that allows the user to rent a book/cd to a patron
and return the book/cd back into my library. The concept
being the user chooses the patron and resource from combo
boxes and they are linked in a detail/master table. I
don't know how to do this and desperately need some help.
Any help you can give me would be greatly appreciated. I
am willing to send a copy of the .mdb to anyone requiring
more information. Samples from anyone who has desinged a
similar application would also be helpful. I hope to hear
