Form usability

Hi, I am new to forms and have a question about whether what i am trying to
do is possible.

I want to create a survey form with radio buttons, drop downs, text etc to
email to various customers and then use the data that is returned. Can this
be done without the customer having to publish the form locally?

Yes, it can be done if (a) the customers all have Outlook and (b) you send
the customers the form as an .oft file attachment. The form should be
preaddressed to return it to you. The customer will run the .oft file, fill
in the desired information, then click Send.
