Form w/option group & subform

I have an employee table with fields like name, address,
field 1, field 2, field 3. Fields 1, 2 & 3 are formated
as Yes/No.

I would like to set up a form where a user will select
either yes or no for the field 1, field 2 and field 3
using an option group for fields 1 to 3 and have the names
that meet the criteria show up in a subform.

I just can't seem to get it to work.
