Form Working, but can't get form fields to line up

I have a form that is working great thanks to help from this forum, but I
can't figure out how to get the form fields to line up. When I hit tab when
I am in front of a field, it brings me out of the form. When I hit the space
bar, I get the field to move, but they will not line up to one another
because the character spaces in the field labels are all different sizes.

The simplest way would be to insert a table into the form, and place your
fields into the table cells.

Your form looks fine with the field titles right aligned.
