Form works when sent to myself but is blank when sending to others

Okay, I got it to be visible to others but they would like to see all the
original email when hitting the reply button the same way as a normal message.

In other words, when they send an email to a customer, they would like to go
to sent items, hit reply on their sent email and have the original message
including...and this is the important part....the "time/date" stamp. Then,
they can copy the whole email and paste it into a database.

That would show proof that they sent it, who they sent it to and what time
they sent it.

For some reason, it doesn't show up when using the form I created, any idea
why?

No no no, You must've misunderstood me. Here's what's happening...

People that I work with want to be able to send an email to others in the
company then

a) Go to their Sent Items folder
b) Open up the message that they just sent
c) Hit reply on the message they just sent (they're not going to actually
reply, the only reason for them hitting reply is that is shows a "Time/Date"
stamp)
d) copy the sent email (with the time/date stamp) into another database
application.
e) All default messages (not from a form) do this perfectly everytime, but
when doing it as a form, the "Time/date" information is there, but the body
and data in the form that was sent is gone, thereby not keeping the full
integrity of the email sent.

Note that if I hit forward, the opposite is true, the body of the email is
there and the time/date stamp is gone.

See if going to the Actions tab in your custom form in design mode and
setting the reply and reply all actions to open your form instead of using a
standard form helps with that.
