Format a date as mm/yyyy

I have an Outlook form with a date on it. I chose the
format in the properties that looked like 8/2004 (it is
august 2004 right now). When I put data into this when
the form is running, I do not get that format. For
example, when I enter 09/2004 it changes to 8/2004. When
I enter 09/2005 it changes to 8/2005. How can I change
this so that it will accept the mm/yyyy format?
