Format Change Applies to Whole Document

In certain documents, when I change the formatting of a
paragraph (like bolding it), the formatting change
applies to the whole document. If I hit control-z,
the "whole document" formatting change is removed from
document, leaving the change I wanted. If I hit control-
z again, the change I wanted is also removed.

What is it about these documents that causes this
annoying "whole document" formatting?
