Format for date in Word 2003

In a Word document I used to be able to click on Insert/Date and Time/ and a series of choices would appear. The third item on the list was in the format: February 22, 2005. I was very used to going through this simple exercise whenever I began a letter to put the date at the top.

However, for reasons unknown, my choice of February 22, 2005 is now gone. Gone completely! I can choose various formats such as 22 Feb '05 or 02/22/05 etc. Does anybody have ANY idea of what happened?

To try to correct I have already reinstalled Office 2003.

I have lost the ability to create custom fields. The "Add" button
remains gray. I wonder if these are related...
