I made a form with the wizard to link to my table.
I have a switch board button that opens the form in add mode (to add new
issues)
I also have a switch board button that should open the same form in edit
mode to review all entries.

Yet the form only opens in add mode.

Check on the properties dialog for the form, on the data tab. Look for the
Data Entry property, make sure it is set to No.
Try opening from the switchboard and see if that fixes the problem.
