Forms Control Box Grayed Out

I want to create a updated appointment template, but when I try to design a
form based on the original (or a copy of) the appointment template, the
control box icon is grayed out.

You can't put anything on the appointment tab. Make your changes on a back
tab; and, no you can't put a second notes or body control on the form.
