Forms Explained Please

I am trying to do a simple form which would be included in my organisation
it. Its a straight forward yes no to certain questions but i am having
trouble with it for hours has anyone got instructions on how i would do this,
like what is the differance to compose and read do they have to be the same
and were do i publish it, org or personel and will it work externaly any
suggestion would be great.

First of all, unless you are working in an organization that has Exchange as its mail server AND you have permission to publish a form to the Organizational Forms library on that server, you will not be able to conduct a survey using an Outlook form. Therefore, check with your system administrator before you go any farther down the forms road.

Whether you use one layout or separate compose and read layouts is a design choice for you to make. Think about what you want the users to see.

The compose page is what the user sees when he/she attempts to create a new
data item from the form. The read page is what the recipient sees when she/he
opens the data item from the inbox. Unless you have a specific reason for
using a split form, you should not use it. Make sure that, on the forms menu,
you have unchecked the "Separate Read Page" option.
org or personel and will it work externaly
You should publish to the Organizational Forms Library if you want anybody
else to access the form. No it will not work externally, unless you send the
form as an attachment and have the external recipients install it on their
Outlook client computers. At your skill level, you should assume that it does
not work with external recipients.

Go to www.outlookcode.com and search for an example of a custom form with
check boxes and yes/no custom fields. This site, and www.slipstick.com, are
the primary sites for Outlook and Exchange information.
