Forms for daily journal

I supervise more than 10 people & the work they do has short term
deadlines. Each day, I have them create a daily log in a calendar that I
created in a public folder. Right now, they simple double click on the date
and an event form pops up. They enter their name as the title and set a few
more odds & ends, and enter their daily stuff, item by item.
Each day, I review what they have entered so I can keep in touch with where
they are in their work, and still have time to do my own work.

I have created a word document that has an outline I'd like them to follow;
the outline makes it possible for me to find & read the info I need on
their work, and it helps to organize their thoughts so they remember what
to enter in the daily log.

How do I make this Word.doc an Outloook form?

Obviously, no experience with forms in Outlook.
Thank you in advance for any and all help.

Did you try copying and pasting from the Word doc into the body of an
appointment item? You could then publish that as a form to the folder and
make it the folder's default form.
