Forms for Report Parameters

Is there a way to design a pop-up form to choose/select
criteria for a report or set the criteria in the query and
then run the report? I am ok with writing macros, but can
not find any clear instructions on how to do this...

Access comes with the Northwind sample database.
Take a look at the 'Sales by Year' report.
It uses a form for date selection.
