Forms help please

Hi,

How can I do this

I need to create a form with 8 rows

ex. Uric
2 - B.U.N
3 - Sodium
etc.
I need to give the user columns to enter dates when testing has been done,
this can go on for 10 - 15 years.

is there a way to add a control (Text Box) based on the number of date
fileds? and how do I maintain the 8 rows to show up when form is open if the
form is a Datasheet view.

This is more like a flow sheet. or you could say a spreedsheet, but I don't
want to use Excel spreadsheet
