Forms- how to reset and prepare them to be sent to other people

I have created a questionnaire using the control toolbox - putting the
questions and then tick boxes/radio buttons
I want to reset the form i.e. clear all the fields from when i was testing
it so that i can either print it or email it to people to fill it in so it is
blank
I understand i also have to protect the document in such a way that it can
be filled in?
If i can get it emailed to people who fill it in do they have to save it as
a different file to send it back?
Also i have not put the options like yes or no in any table i literally
pressed the buttons and renamed each one underneath each question.

Protected forms are created with fields from the Forms toolbar, not controls
from the Control Toolbox. If you protect a form created with ActiveX
controls, you will not be able to unprotect it. See "WD2002: Can't Unprotect
