"Forms" in Word (2003)

I have designed a two-page FORM in Word(2003) to automate the collection of
user data.

For printing's sake and for keeping the two pages together, I want to show /
display / print identifying information on both pages (like name, phone
number, email, etc).

But I don't want to require the users of my form to enter their names and
phones etc info twice.

Please HELP.

Need more info, please ask!!
