Forms in Word Document

I want to prepare a proposal document everytime for different customers. I'm
having a common template in which I'm making Proposal.
I want to incorporate a Form which should have a textbox like Customer name,
Address, Value of Proposal, Technical Input etc., so that once if i enter the
data in the form, those data should be incorporated at the correct place of
the document.
Is it possible to do like this?

Please guide me.
