Forms not being received

We created a custom form with some coding in it. We are using Outlook 2003.
The form when SEND was clicked would go to the person that it was addressed
to. We added a few more objects. When we click on SEND, the form looks
like it is going and no error messages are populated, but the form does not
reach the recipient. Any ideas why a form would stop working?

What do you mean by "the form does not reach the recipient"? A "form" is the
UI/code template an item, in this case a mail message. It is the message that
is sent, not the form. I can only assume that you mean that the recipient got
a message but it didn't look like the form?

Did you design a read page layout to match the compose page layout? Did you
publish the form to the Organizational Forms library on your Exchange server
with the "send form definition with item" box on the (Properties) page
unchecked? Those are both prerequisites for making a message form work within
an organization.
