Forms or Templates in OUTLOOK 2003

I am trying to make a form or template for our Purchasing Agent for
expediting order. I want a simple fill in the blank form/template that the
purchasing agent can fill in the Purchase order number, Scheduled Date of
shipment and requested shipment date. The reciepent would then fill in the a
different set of information such as Confirmed Ship Date and Scheduled Ship
Date.

I made a form but when i fill in the blanks and send it the information is
erased when the receipient receives the email and vice versa.

How do I make this work?

Did you publish the form to the Organizational Forms library on your Exchange server? Did you click the Edit Read Page button and create a read layout for the recipient to see?

I did edit the Read Page and create a read layout for the receipent to see,
but when they see it my original user input is erased. The form can not be
installed because you do not have owner permissions." I have contact our IT
to change permissions.

Now When i created the fields for user input I used a text box. Inside the
text box for example "PLEASE CONFIRM SHIP DATE ON WIT PURCHASE ORDER:________"
The user is to fill in the blank in the text box, which when testing I can
but the input is erased when sent.

Any ideas?

As far as publishing on the Organizational Forms Library, I get a an error
saying: Unable to successfully publish the form due to a MAPI error

If you cannot publish to Org Forms, a form with custom fields will be unusable. (unless you can persuade all recipients to publish). Perhaps your Exchange administrator has not created the Org Forms library or given you permission to publish there.

Any control that you want to use to store data persistently must be bound to an Outlook property. Do this on the control's Properties dialog, on the Value tab.

So if it does get published on our server, will outside vendors be able to
see the information?

If not how can I accomplish this?

No, there is no practical solution for using Outlook custom forms to communicate with people outside your Exchange organization. Consider using a different solution, such as InfoPath or web forms.
