Fill-in Form General Questions

I have a form that has several tables with 4/5 rows. The 1st row is the title
and all the other lines have a fill in text field.

I need a space for notes (free text, including formatting, etc). I created a
2 row table (header and 1 blank line) which I sectioned off and left
unprotected so users can type in free form text. It works ok, but I have a
couple of issues I need help with:

1. I would like to fix the title row so they can't modify it

2. This table is at the end of a page, I want the for to expand to allow
more text, which it does. However, when I click on "don't allow to break row"
it only moves the 2nd row and not the header row. I wanna move the whole
table to the next page.

3. Maybe there is another way of doing this without a table and just
"drawing" a box around this field so that it looks like the other tables, but
it doesn't have all the restrictions...

Format the heading row as "Keep with next" (Format | Paragraph | Line and
Page Breaks).
