Freight Bill of Lading Report

Anyone ever created a generic Bill of Lading report or seen one they would
care to share?

Just want to cut some corners and not re-create the whole wheel.

Many trucking companies have these forms available for download in PDF format.
If you prefer, you might be able to get them to fax or email a copy.
