From is blank after sending

I have created a form where the 'To' field's inital value is set based on a
option button list.

The 'To' field appears to populate OK, however the 'From' field is blank,
which causes the email to be rejected by the exchange server.

When the 'To' field has the initial value removed, the 'From' field is
populated like normal and the email goes through.

How can I fix the form so the 'From' field is populated correctly? Would it
make a difference if the form was published under Organisational Forms?
