Front Page form not working at all now.

Back on November 16, 2006 I posted a request for help "Front Page Submit
Button Issues" and had great feed back and the form was working perfectly.

Our ISP updated their server at the beginning of this year and the form
stopped working.
This is the message that comes up when the "Submit" button is clicked...

Not Found
The requested URL was not found on this server.
Apache Server

They told us to republish the form - which we did - and it's still not
working.
They claim that they will not support Front Page much longer and we should
look at alternatives for form formats.

Shouldn't we be able to use the form because Front Page Extensions are
listed as being available from this particular ISP?
Nothing was changed on our form. We were just told one day by potential
customers that it wasn't working and had no idea what what was going on.

Any input - again - would be appreciated.

1) What "update" did they do?
2) I'm afraid you're going to have to trouble shoot the issue with your ISP.
They certainly have changed something.
3) You should be able to continue using the FP extensions until they no
longer provide support. That's their choice. Support by the creator of the
extensions has been discontinued, and they are no longer available for
download.

There is a second form bot in the very bottom of your page in code view.

This is from an email the ISP sent notifying us about the update/change...

XXXXX XXX has moved your website to one of our new Dual Core Xeon servers.
This will provide a faster and more reliable server for your web pages and
email.

The upgrade is transparent and email access will be the same as your old
server.

NO ACTION IS REQUIRED BY YOU.

We tried to ask for assistance but basically that is when they told us to
re-publish the form and possibly even go with php scripts.

The key is how did they move your site! It needed to be published and the FP extensions must be
configured to process email on the new server by the web host.

Stupid Question...

What should the file path be?
This is what's driving me crazy after looking over the layout over & over
again.
I didn't modify the form that was working and now I'm having these issues.

Better yet...

I found the original form I used that had the original problem from November.
(Clicking the "Submit" button and the email opened up.)
I removed the line that was causing the original problem.
In November, after I removed that line, the problem was corrected.

I renamed the form as to not confuse myself or others and it is here...

However, when the "Submit" button is clicked, then following message
appears...

Cannot run the FrontPage Server Extensions on this page:
"http://www.burloaknannies.ca/fam_quest.htm"

To me, this would indicate that it is an issue on the ISP end of things and
no longer a form issue.
Am I correct to think that?

If I already have "form_results.txt" set up, wouldn't that act the same as
"Feedback.txt" or does it specifically need to be "Feedback.txt"?
