Front Page forms requiring password

Hello.

I've been going back and forth with my hosting service over this issue and
no one has come up with a solution. I've setup a basic form on my site as an
example to what is happening.

Sumitting this form yields a request for a username and password. I've
checked that the _private folder has write permissions set. Can someone
please explain why this may be happening? The site "used" to work fine and
one day it didn't. Not sure what this means.

I don't see any problems using that link. The form submitted and a
confirmation page appeared. The confirmation page did not show any
submitted data, but did show the field headings.

Your _private directory is not the only part of the submission process that
requires permissions. Another common cause is the permissions on the
FrontPage Server Extensions are incorrect. Because the FP Server Extensions
are actually processing the form, if their permissions are incorrect it can
result in a permissions dialog.

Strange... for some reason, on my screen, I'm still getting that error message.
Could someone do me a favor and please check this one?

Maybe I'm just losing my mind. Not sure why it would work fine on
another computer but not this one.
