Frontpage and javascript help needed

I am an avid MS Frontpage user and want to use a form in
one of my sites that calculates. Someone on this
newsground told me ages ago to check out
www.frontpageforms.com, which I did and got a form there
that calculates (made by Texas Web Developers) called java
order form sample but I am unsure how to get it to work ie,

- how to input the email recipient
- how to change the shipping prices and I would especially
like to put in a drop down menu for 3 shipping options and
still get it to calculate.

Is there someone out there that can seriously help me with
some good old step by step instructions to get this going?
I feel that noone can help me or wants to help me but I
desperately need to sort this out and I am stuck until I do
get some 'real' help on this.

Or if someone thinks I should use a better one and can also
explain that, that would be fine too.

Yes, I have. The form itself was actually created by a
guy who works for Texas Web Developers but he did it for
the site www.frontpageforms.com. I have contacted both
parties and the one that created it said he can't (or
won't) help me as he now designs/builds sites for
businesses. In other words, he is one of those designers
who won't help other designers. The other guy has been
good but he can't help me any further as he doesn't know
javascript very well.

So, I don't really know what to do now. I don't have to
use their form really. I just need something quickly now
that works ie, calculates etc and one that I can put
easily into Frontpage and get it to go the email recipient
etc.
