Frontpage email form is sending an empty fields help

please i have designed email form using FP and i have set the FP
extensions correctly to my server as you have told me but now am
testing my form the problem is that its working correctly and sending
data to my inbox but also the fields are empty theres nothing except
the filed names and nothing from what i have entered to the fields been
sent to my inbox.

please help me solve this problem

entering any data to fileds are not sent to me plz help me
