FrontPage Forms are not working

I am running IIS 6.0 on a Windows 2003 Server with FrontPage 2002
Extensions installed.
I uninstalled and reinstalled Extensions.
I checked the Server extensions from the default web page task menu.
I checked Server health and told it to repair. It tightened security
but thats it.

I am concerned that it is a write permission problem somewhere in the
default web, _Private folder or the _vti_bin folder. Here is the
scenario: I created a straightforward form that asks a number of
questions which should save the results in the default formslt.txt
file. Each time I run the form, I am able to tab around and fill in
the blanks. When I submit however, I get
FrontPage Error.
User: please report details to this site's webmaster.

Webmaster: please see the server's application event log for more
details.

When I open the formslt.txt file, it is empty. After reading a number
of posts, I built a default feedback form using the template. It gives
me the same error with no data posted to its text file.

Ideas?

What is the error in the server's application event log? It's hard to know
exactly what's going on without the error as it could be any number of
items. If you can post a copy of the error in the event log that would help
narrow down the issue.

I see no errors in the Application log. I did a find and included
Frontpage in the description. The only frontpage entries I find are
where I have uninstalled and reinstalled the extensions.

I just ran the form and when I submitted, I got back the FrontPage
error, Here is the url if that helps,
http://www.aqci.com/_vti_bin/shtml.dll/medicalcoders.htm.
I immediately looked into the application event log and see no entries
for this date and the time I submitted.
