FrontPage2000 Bulletin Board?

Is it possible to create just a bulletin board (as opposed to a discussion
web) so that customers can post their comments regarding my site and
products?
I would like the following included in the bulletin board.

Name:____________________
City/State/Country:______________
Website address: _____________________
Comments:

Please advise

i found the answer to it. i was looking for "bulletin board" and it's under
feedback page. sorry, i'm not very experienced at this.
