Full context display for cross-references

"Full context" cross-references to outline-numbered lists include
higher numbering levels, even if the number format does not.

For example, in an outline-numbered list such as the following:

1) one
a) one.one
b) one.two
2) two
a) two.one
b) two.two

a "full context" reference to the item I called "two.one" would
display as 2)a).

For outline numbering that includes higher levels, such as 1, 1.1,
1.1.1 numbers, the "full context" option has no effect (for obvious
reasons).
