Getting a username/password prompt when submitting form

I have a form page at
http://www.soffoundation.com/application_2.htm When anyone attempts to
submit the form to the designated email address they're prompted to enter a
user
name/password.

I enquired about permissions from my hosting service
and
they told me that it is a scripting issue. They told me that they checked
the permissions in the _private file and all looked well. So if this is an
issue with FrontPage or my HTML coding, where would I begin to look in
correcting this problem? I'm running FP 2002. Thanking you in advance for
any help with this.

1. I can't see where you've designated an upload directory for the photo.
2. You have made the submit button a mailto: link. That's not what the submit button is for. The
form will email itself to the email address you have specified in the form properties contained in
the opening web bot section of the form.
3. I strongly urge you to *not* use the space bar to position content, but to use the left right
and center icons on the toolbar.
All of these

p;    &nbs

placed throughout the page are going to cause your page content to be shifted when displayed in
browsers with different font sizesettings.

I'm pretty sure it's not a FrontPage issue from a scripting (of the page or
extensions) point of view.

have your server admins check and if necessary reapply the FrontPage server
extensions.

are you publishing using the prescribed method or doing something "off the
menu"?

The File Upload component is being used and not all hosts support it, plus it requires the FP 2002 SE to be configured per
