Looks like your video driver is the problem - - module 3DFX16V3.DRV.

I did a google search on 3DFX16V3.DRV and the first hit I got was
http://support.microsoft.com/default.aspx?kbid=300912, which explains that your
video driver is in need of being updated. See the help in that link.

Thats pretty common problem, your video card driver is old and out of date
3dfx has also gone down the tubes many years ago, welcome to the 21st
Century

Luckily for you some other people still also use this card in their ancient
machines

Download the latest driver for it from
http://www.falconfly.de/voodoo3.htm

You want the 3dfx Voodoo3 v1.07.00 whql certified

the mvps also have a link for the MS KB article which they will pull out of
the air

note:- sometimes you think you have installed the driver but you haven't...
