Getting data from list

I want to be able to transfer names from a list in
worksheet 2 to a form in worksheet 1, i have about 40
names in list and need to select 18 for form, i tried the
IF formula but i get a #Value error, I have a small demo
i can email to you if you can help, plleeaassee help i'm
tearing my hair out

If your form has a code number for a name and the list has both, then the
VLOOKUP function might do what you want, or if you want to just type a name
and verify if it's on the list then VLOOKUP will do that too......
