Getting Names and Addresses into Address Book / Contact List

I have a rather lengthy list of names and addresses in the following format:

Mr. John Doe
123 Main St.
Anytown AZ, 85002

Mrs. Jane Smith
444 W. Pine Rd
River Bend, OH 92345

Mr. Tom Jones
789 Mountian View Ln.
Hilltop, IN 56732

Is there any fairly simple way to put these names and address into an MS
Word Address Book / Contact List?

For the purposes of mail merge, Word will use the list as it stands. If you
want to incorporate the addresses into Outlook's contacts list, you will
need to convert to a comma or tab delimited format - see
