Grouping by Date and totaling.

Hello,

I posted a similar message last week, but i still can't
figure this out. I have a qry that collects the current
date and has a few check box for some question asked in a
form. I would like to make a report totaling the check
boxes to the month. Like This:

------------------------------------------------------
Chkbox1 Chkbox2 Chkbox3 etc..
Jan 1 2 4
Feb 0 5 10
Mar 3 13 14
Apr 12 23 34
May 23 2 1
Jun 233 2 1
Jul etc..
Aug
Sep
Oct
Nov
Dec

If this possible any help would be great. I'm clueless
when it come to reports.
