Has anyone been able to create a meaningful data model report?

I cannot find any help in Visio or online that enables me to create a
meaningful report from a data model developed in Visio 2003 Pro. I'd like to
report Entity Types, Attributes (column names), and Notes for columns that
include my valid values.

Has anyone found documentation for this or determined how to do it on their
own?
