has anyone code for a key pad for macros ?

I have 12 macros which are either special symbols or short phrases for
insertion in text input into form fields. Is there anywhere when I can find
code for setting up a 12 key pad that I can put into a form so that each key
in the pad acts like a control button and runs the macro inserting the words
etc I want. I want to do this as having the 12 keys (or control buttons)
linked into one would be easier than having a macro tool bar or 12 separate
control buttons.
